Kauai (Hawaii) (Reuters) - U.S. authorities said on Thursday that responsible health Hawaii that received national attention when he issued a copy of the birth certificate of President Barack Obama in 2011 was killed in a plane crash off the island of Molokai.

A spokesman for the National Transportation Safety Board that the agency is investigating the incident and will issue a preliminary report within 10 days to 14 days.
Fode was issued amid accusations that Obama was not born in the United States a copy of the original certificate of his birth in Hawaii.
She said at the time he was "in recognition of your status as president of the United States" is the exception to the policy which requires that management is not only to issue a certified copy of a computer and through the birth certificate.
Obama then issued a copy of the original birth certificate in response to allegations of widespread that he was not born in the United States.
